Your path to a supportive home

Welcome to Embracing Your Comfort & Care Homes. We are committed to making the process of finding a safe and comfortable home as clear and straightforward as possible. Discover the steps we take to ensure a personalized and welcoming transition for every resident.

Step one: initial inquiry

The very first step to getting started with Embracing Your Comfort & Care Homes is to complete our Client Referral & Placement Inquiry Form. This form allows the individual, a family member, guardian, case manager, social worker, or other referral professional to provide essential information about the prospective resident. We gather details about their housing needs, the level of support required, the funding source, and their preferred placement. Once submitted, our dedicated team will carefully review the information provided, confirm current availability in our homes, and contact the referral source. This initial contact will discuss eligibility, outline the next steps in the process, and, if appropriate, schedule a consultation or a tour of our facilities.

Navigating the next steps

After submitting the referral or placement inquiry, our team will thoroughly review the information to gain a deeper understanding of the individual's housing and support needs. We will then reach out to the individual, family, guardian, case manager, or referral professional to discuss the referral, verify funding and any necessary documentation, and determine if one of our available homes is the right fit. If appropriate, we will schedule a consultation, an interview, or a home tour to allow you to experience our welcoming environment firsthand. Before admission, we may conduct a pre-admission assessment and review all relevant care or support documents. Once placement is approved and all necessary arrangements are finalized, we will coordinate a move-in date and provide comprehensive assistance to ensure the transition into their new home is as comfortable and welcoming as possible. Our goal throughout this process is to make placement simple, supportive, and personalized, ensuring each resident is matched with an environment that truly meets their individual needs.

To ensure a smooth and efficient intake process, please have the following key information and documents ready. We will also need contact information for their guardian, case manager, emergency contacts, and payee. Details about funding sources, monthly income or housing budget, payment responsibility, and confirmation of approved funding are also essential. Finally, please provide any relevant assessments, guardianship documents, medication lists, behavioral or crisis plans, Individual Plans of Support (IPOS), and discharge documents.
